Deep cover did not secure classified material

In Russia’s 2010 Illegals Program, agents posed as ordinary Americans and built contacts with policy figures. The FBI exposed them before they obtained classified material, despite their long-term cover.

Cuba became the first country recognized for eliminating mother-to-child HIV transmission

In 2015, the World Health Organization recognized Cuba as the first country to achieve validated elimination of mother-to-child transmission of HIV and syphilis as public-health problems. This did not mean transmission had become biologically impossible or that no cases could occur. It meant prevention, testing and treatment had reduced transmission to very low levels under established public-health criteria.

A shut-down nuclear reactor still produces a lot of heat

Stopping nuclear fission does not immediately stop a reactor from producing heat. Radioactive materials continue to decay, initially producing more than 6% of the reactor's normal heat. Cooling must continue or the fuel can overheat and melt.
